Fee schedule/No Surprises Billing
Kayla Clary Nutrition LLC is an in-network provider with BCBS insurance which may (or may not) cover some or all of your visits.
- To find out if your policy covers nutrition visits, please call the number on the back of your insurance card and ask for coverage for seeing a dietitian. If asked for CPT codes, nutrition visits are billed under 97802 (initial visit) and 97803 (follow up visit).
For all other insurances, we will be an out- of-network provider, and can provide you with a super bill to submit to your insurance. Do note that this rate will be billed at the insurance reimbursement rate, which is generally higher than the self-pay rate.

If you do not have insurance that covers nutrition visits, self pay rates are as follows (can use HSA/FSA cards for these visits):
Initial Nutrition assessment (60 min):
-General Nutrition: $119
-Tube Feeding: $139
Follow up nutrition visit (30min):
-General Nutrition: $55
-Tube Feeding: $70
